ProForma, a powerful form builder for Jira, allows you to easily create fields that capture the information you need. It allows you to easily customize the information you receive when users create issues. Business teams create forms that ask for the information they need, ensuring that all required data is submitted with the issue.

You can design forms quickly and easily either starting from scratch or utizing the 250+ form templates.

How do ProForma and Jira Core/ Software Work?

         The Assigned Team Member (agent) sets up a ProForma and Jira issue: 

  1. Create a form that collects the information needed for a given business process.
  2. Create a Jira issue related to the process/request.
  3. Add the form to the issue.

    Internal Customer
  4.  Goes to the issue and opens the embedded form.
  5.  Fills out and submits the form.The Approver
  6. Views the issue and the submitted form and gives their approval (either by transitioning the issue or by adding and completing an approval form).The Assigned Team Member
  7. If necessary, customers and service desk agents can further discuss the request via the “Comments” tab on the issue.
  8. The issue is reviewed, acted on and completed.

How Do You Set It Up?

Step 1: Create form

Forms can be created from scratch, from a copy of an existing form, or from a template in the template library. You can add or delete questions as needed.

Step 2: Create the issue

Create an issue based upon the task, request or process you need to carry out.

Step 3: Add the form to the issue.

Now your internal customer can come to the issue and complete the form, giving you all of the information needed.
